Service Description

We offer intensive diabetes care management, regular in-person care and assist in building knowledge and skills of the person with diabetes to improve A1C and decrease hospitalizations. - Height, weight and BMI calculation - Blood pressure - Interpreting results of blood glucose monitoring and ketone testing - Tailor recommendations based on laboratory values such as lipid profile, A1C, and vitamin/mineral deficiencies - Physical examination - Medical history includes medical diagnosis and comorbidities as well as current medications - Assessment of knowledge, skills, and coping level to individualize the intervention - Assessment of emergency preparedness, including availability of glucagon to parents, roommates, and significant others knowledgeable about administering it; wearing of diabetes identification; its impact in driving - A plan for continuity of care is critical for successful transition from pediatric team to adult care team - Frequency and treatment of hypoglycemia and hyperglycemia; presence of hypoglycemia unawareness in relation to: -> Meal plan -> Physical activity and exercise habits -> Psychosocial factors that may influence diabetes management; use of tobacco, alcohol, and/or recreational drugs
